Thank you for your interest in the Bloomington Cremation Society. By submitting your information, you are not obligated to use our service. You will receive an Advanced Directives kit, containing forms for Appointment of Healthcare Representative and a Living Will.
   You will also receive a biographical information form, useful in composing an obituary, in addition to a Final Disposition form, detailing your selection of cremation and any additional services you desire. These can be retained by you, or returned to the Bloomington Cremation Society.
